Age UK Herts: Hospital and Community Navigation Service

The free Hospital and Community Navigation Service is available throughout Hertfordshire to help patients recently discharged from hospital and those who have recently been unwell. We can provide wellbeing checks for people in their own homes, help to develop motivation, confidence and resilience, link people to support with benefits, debt and finance and help with solutions to loneliness and social isolation. We can also help people affected by hoarding.

Who can use this service?

People who have recently been unwell or those in the community who need help to manage their health and wellbeing.

How are services delivered?

Initial contact to 0300 123 4044 and then we can direct you to the relevant team. We are working in partnership with seven organisations across Hertfordshire.
